I hereby authorize the Company upon execution of this consent form, to investigate the information contained in my employment application and any other background information (which may include but is not limited to my creditworthiness, credit standing, motor vehicle reports. credit capacity, criminal background, driving background, character, general reputation. personal characteristics. and mode of living). for the purpose of obtaining information relevant to my qualificatK>ns for employment, and unless prohibited by applicable law, my continued employment, retention. prorrotion. demotion, or any other employment purpose. I understand and agree that the Company or an outside consumer reporting agency may secure a consumer report or an investigative consumer report, as defined in the federal Fair Credit Reporting Act and any applicable state or local laws, from an outside consumer reporting agency. I hereby release employers. schools, health care providers and other persons from all llablltty In responding to inquiries and releasing information In connection with my application. I understand that false or misleading information given In my application or interview (s) may result in discharge. I understand, also. that I am required to abide by all rules and regulations of SandBox Transportation, LLC. I understand that information I provide regarding current and/or previous employers may be used, and those employer (s) will be contacted, for the purpose of investigating my safety performance history as required by 49 CFR 391.23 (d) and (e). I have reviewed and understand all terms in the Disclosure seetion. I understand that I have the right to: 

• Review information provided by previous employers
• Have errors in the infonnation corrected by previous employers and for those previous em􀂁oyers to resend the corrected information to the prospective employer; and 
• Have a rebuttal statement attached to the alleged erroneous information, if the previous employer(s) and I cannot agree on the accuracy of the information.
